No, that’s not what I mean. If you lost all your friends from one day to another, do you think „it’s okay, I’ll just make new friends.“? Of course sometimes we don’t have much of a choice (like when your parents move as a kid and you have to move), but in this case the choice was there. Whether I make new friends or not, the pain of losing old friends is still there after all.
The choice should have been obvious. Don't take the drug and live your life as male like he intended in the first episode. He would have been an adult, could have had a job, earn money, play his games, meet people and everything else he did up until now as a girl and he tossed his masculinity away and adulthood for nothing. If I was an adult and I had to relive my middle school I have had depression.
From what I know, the dude was an anti-social unemployed shut-in before the events of the series. It’s unlikely that he would have gotten his life on the right track if left to himself

Look, if Mahiro really felt so uncomfortable in the girl’s body, I think he would be willing to give up his friends as a girl in order to go back, but it doesn’t look like he minds that much at all as we see in this episode. Other people in his place might choose differently, and that’s fine.
if he was so content why did he have second thoughts before drinking it this episode? The drug ain't going to last forever anyway( lasted like 2 months at a time. Is he going to take the drug every 2 months for the rest of his life until he dies at the age of 100?)
