While we wait for the OP to respond, here's what I think:
Have you visited India before? Visa number? Date?
Since you haven't visited India on a visa before, the answer will be "No".
And since the answer will be no, you don't need to mention visa number or date.
I agree the eVisa form should give more clarity on what they mean by this.
(since I born in India, and recently received a canadian passport-want to surrender Indian passport in India).
I was in the same boat as you a few years ago, and got my eVisa twice since then. The first time around I did select "No".
Are we allowed to travel/obtain an Indian tourist visa-prior to submitting the surrender certificate of Indian passport?
Once you obtain citizenship of any other country, you automatically don't remain an Indian citizen, submission of Indian passport is due. So you can take a tourist visa on Canadian passport.
I don't know if you can still travel on the Indian passport, I guess there's allowance for emergency, but don't quote me on that, I never researched or tried.
