If I just fetch `survey.questions`, I've got a bunch of questions in no particular order. How would I convert this to a depth-first ordering of the tree in Ruby? I couldn't think of a way (unless we changed the order_number to store more information).

We *do* have a root survey_id for all these elements.
The problem is that the order_number is local to each sub-question level, so we couldn't do an `order by order_number` up top.
