Your post here does a great job of addressing the need to address relationships across the entire Scrum team, including the extended team. What I see missing is that tools and processes for the entire Scrum team need to be addressed. Lean recognize the experience front line workers have and to give them the problem solving skills to remove barriers preventing them from delivering exceptional service.
