Are multiple people managing the form. It is likely that other users may have also created the rules that may be causing duplicate notifications.

Are you CC'ing or BCC'ing anyone in Mail Merge. I am asking this because Gmail counts every CC or BCC as a separate email and thus it counts towards your email quota.

You can add the generic attachments to the Gmail draft template and they'll be added in every outgoing message through Mail Merge.

The limit is 100 per day if have the premium license for Mail merge else the limit is 50 per day.
