on the issue of clean water, there are some very cheap medications which destroy water-bourne worms in children which greatly improve life expectancy. The worms take 10 years to multiply in the body to a dangerous level - so a tablet every 10 years or so is enough. More needs to be done on these low level, low cost solutions
