When I first began my blogs, only my wife and a few family members used to comment and I used to go for days without any comments..Then, I began visiting other blogs and leaving comments on a regular and began telling people about my blog and before I knew it...the comments started coming in..I average about twenty comments a day. By the time I launched my second blog ("Escapades") I was like a household name and it helped that blog get readers...My wife just launched her own blog recently and I'm watching her go through the same growing pains regarding comments that I went through...

It goes down to build quality and design aesthetics. When Apple does come with the same eye popping aesthetics with CORRECT technical design (ex., proper cooling solutions in their oh so slim macbook pro) plus better bootcamp, I will be one of those people lining up to get a Mac. But if aesthetics and build quality plus the pogi-points is concerned, Apple does deliver.

(Almost) exactly the same as "Bubble & Squeak", which everyone living in London from the days of the War and the subsequent years of rationing will recognise. That was made with the remains of the one big dinner of the week, the Sunday roast, where the left over greens & potatoes were mixed together & fried in dripping. Wonderful!

Greydon-you nailed it. Religion exists because people are afraid of death. It is also a control mechanism. Sounds like you made a great argument with your ex and your brother's girlfriend. As you you have learned, you can't reason with these people, because their "faith" teaches them not to believe reason. As a white man, I don't understand why any African American person (or Native American or Hispanic person) would be a Christian. It was under the cloak of Christianity that their ancestors were oppressed for so long. I commend you for seeking out and finding the truth, and defending your beliefs so well. My name is Keith I am one of those Christian Universalists. I think the Bible is not perfect. But I also get a lot of good out of it. I am lucky that my Christian "Baptist" Parents were not legalistic. So I grew up pretty happy. I used to be an atheist when I was a kid but not as well informed as you people. I have found that the atheists I run into know more about the Bible than the average christian. Ironic. Don't ya think? Anyway I have been down the Calvinism, Arminian road and now I am a Universalist. Who knows maybe I'll be an agnostic next. I don't think I will ever be an atheist. It just seems to me the complexity of the universe tells me there is a creator. Not a GOOD Christian God or a Bible God. But at least a creator. Thank you atheists for challenging christians on our stupid beliefs. We need to be challenged. I am glad we live in a place where Christians are not allowed to burn people at the stake for disagreeing with us. Anyway I thought you would get a kick out of this: I was thinking one day and I made a list of BAD things God made and I don't know why. What do you guys think about christian theodicy? And do atheists have a theodicy? Here is my list. Is is not complete it just came to mind. What do you think?

what I noticed especailly about this past election was the amount of violence on a grass-roots level. We had a lady get her car spray-painted with the words "Bush Zazi" and swasticas (sp?) on her car just for having a Bush sign in her yard. There was another story where some type of Republican office get it's windows shot out by someone in the early morning! I heard several stories like that - and just in this area alone. -Keith

Surely you didn't know about my own difficulties today posting my first photo on my blog! I learned to use Hello, but it's not very obvious to me either! The biggest aha for me was that I cannot upload a photo to an existing post. Rather, you have to first upload a photo, which creates a new posting. Then you can edit that posting to type in text. Opposite the way I think!

They rang me constantly for a week or 2,not sure how many times a day as I was at work at the time.
When they rang me in the evening all i got was a silent call,I dialled 1471,as I do everytime I get in from work and when you call them back you get the message this number cannot be connected or you have dialled an incorrect number.
When I eventually managed to speak to somebody I told them i am not interested in what they are selling before they try to sell me anything.
They still ring me everyday and this is really cheesing me off

I have long been under the opinion that we need to drastiacally restrict our government. McCain is clearly the better choice, given our options. Obama will expand government creating an irreversible government oppression of freedom and prosperity. Most importantly, no one earning less than $100,000 per year should be paying federal income taxes. It contributes nothing to the common good, especially with as much money is wasted on a daily bases by our government. Anyone that claims otherwise is either clueless about economics or is just plain lying. The top priority of the federal government should be to begin eliminating tax brackets from the bottom up and no increases on anyone. The whole soak the "rich" tax mentality is based in nothing more than jealousy and only serves to prey on that ignorance and harm all of us.

Milton Friedman espoused a capitalist fantasy land where the idea of letting the strong survive was the esscence of what he promoted. His policies, which were taken to heart for several years by our goverment, certainly enriched our country, but economically bankrupted multiple coutries in South America and Africa. The only success story that can be claimed is in Chile, where our goverment assisted in ousting the legimately elected president and installing a dictator who brutally enforced the policies of the Chicago School of Economists by stripping the ecountry of subsidized health care and education and "disappearing" many of his political opponents. By the way, Chile now has a public health care system that covers 75% of the population according to WHO statistics, so they apparantly learned along the way that the economics that Milton Friedman espoused were not exactly very wise, and certainly not humanitarian. Either way, I don't think you would have enjoyed living in Chile during the time that Pinochet decided to follow Mr. Freidmans advice.

Not a very good choice to be quoting at this very difficult economic time for many people.

One has to define what is the "best care". If you insist on offering the "Best care" to your community, but only 50% of your community can afford the best, what do you suggest the rest do for themselves? Get substandard care? Why not try to offer the best care to all and try to give up that new BMW you have had your eyes on for quite a while.

Once again, working to give our profession the good name it deserves.

While I agree the AMAs attempt is alot of fluff with no teeth to it,it seems our profession should indeed remain one that takes care of its community when times get tough. The past years of Wall Stret greed have seemingly made everyone jealous of the increible incomes that these guys have made, along with there fellow CEOs, that has made us all feel like we are all being taken advantage of, and should be compensated more generously for our incredible sacrifice. I personally found medical school and residency to be one of the more exciting times of my life and by no means a sacrifice as one of your posters claims. If you choose to take on years of training, do not expect that a big pot of gold awaits you when you get done simply by virtue of the number of years you put in. Otherwise we would have alot of well off English and Philosophy PHDs running around caliming how many rigorous years of training they have and how they sacrificed the best years of their lives.

It is time that we all look to what can make medicine more affordable and equitable for everyone, which is what the current administration that you seem so critical of is attempting to do. Then we won't have to worry about whether our patints can afford the treatments they need or not. The the whole issue of charity care and who delivers it can go away and we can concentrate on improving the overall quality of the health care system rather than trying to figure out how to avoid our responsibility to the under and uninsured.

I want somebody to give us more choices. If insurance companies offered policies that did not include treatment coverage for items and treatments that were not proven to be clinically more effective than the old therapies, then that would be choice. Right now, you get expensive policies that cover everyhting, proven or unproven, that are outrageously expensive.

You deride the goverment for bringing the country to the brink of bankruptcy, but I will bet you cash those checks from Medicare every month. You shold stop being so two faced about this and do fee for service/ no medicare or insurance in the hospital and stop robbing the Medicare National Bank!

And I will say it for the umpteenth time; EVERYONE CAN BUY WHATEVER THEY WANT IN TERMS OF THEIR TREATMENT;GOVERMENT DOES NOT DECIDE FOR YOU WHAT TREATMETNS YOU CAN OR CANNOT HAVE. JUST DON'T EXPECT YOUR INSURANCE COMPANY OR MEDICARE TO PICK UP THE TAB FOR UNPROVEN OR MINIMALLY EFFECTIVE TREATMENTS. Otherwise, we might as well have snake oil salesman sending their bills to Medicare for their unproven remedies.

I will define basic health care for you. Those things that are clinically proven to be beneficial to improving health. Not things that MIGHT improve your health, but only those things we have firm proof of clinical benefit. If we had this rule, we might not have had 20 million kids have there tonsils extracted with no clear medical benefit and we wouldn't have had all these men having TURPs in the 70s and 80s till it was shown not to be MEDICALLY BENEFICIAL!

Nobody suggested that you cannot have your cancer chemotherapy if you want. It is a matter of whether these therapies that are exorbitantly expensive will be covered by insurance, which drives up the cost for everyone and forces more compaines and individuals to drop their coverage. If one wants to pay the 100 grand cost, feel free to buy your drug. Just don't expect someone else to pay for it for you.

Goverments in other countries are making the hard decisions that we seem incapable of here. They are making decisions to provide for the basic health care needs of their citizens before they began buying all the fancy new toys and drugs that provide little improvement over the old therapies. This is here in this country. What about mammography waits in other countries? And furthermore most mammograms are non urgent anyways. And by the way, I just heard a patient say she couldn't get a mammogram at Northwestern without a 4 month wait.

We can trade examples of the imperfections of both systems all day. I beleive a goverment run (or at least the option of a goverment health plan) woul be better than our current extremely expensive and dysfuntional attempt at the free market system.

The reasons you state for the increased cost of an MRI here make no sense. If, as you suggest, it is because of lack of competition on the insurance side, then the usual market response is that the insurer could drive the price of an MRI down in a saturated market of MRI providers (which is what we have).

Now I could accept that insurers are happy to keep the prics higher, because the number they track in being profitable is the percent they pay out in medical claims. The higher the percentage, the less they have to pay stockholders and executives. If the market bears higher prices for insurance, which it has till now, then they stand to make more paying 1000 dollars for an MRI vs 500 dollars. If their medical loss ratio is 80%, then simple math tells us that they get to keep 200 dollars for the 1000 MRI vs 100 dollars for the 500 dollar MRI. Of course they have to keep charging more for medical insurance which seems to have been a successful strategy the past few years. This may explaiun why they seem so willing to pay hospitals more for these type tests when they could be performed for much less cost in an outpatient facility.

Of course Canadians don't WANT to wait for their test. But I can tell you if I refer a patient to a rheumatologist at my hospital, they will be offered an appointmnt in three months. And I can think of several other examples where wait times exist just in my world, which is, by the way, at a very well funded hospital and health care system. Secondly, what about the patients who have limited or no access to cancer screening and the examples of women who do not get proper pap sceening or mammograpy? And your example of colonoscopy waits; I dare you to try and get a screening colonoscopy AT ALL if you have no insurance in this country. You currently have to wait more than 6 months for a mammogram at the county hospital here in Chicago.

As to my oncology friend, length of survival is but one parameter which you have conveniently picked. More important is months of MEANINGFUL survival. Who wants to sign up to live an extra 6 months while being bombarded with highly toxic chemotherapeutic agents while you suffer a slow but painful death? And the NHS in Britain allows you to obtain private healthcare if you wish (but few do) and does not restrict you from receiving any form of therapy unless it is not approved by their equivalent of the FDA, which is no diffrent from what we have here. Nothing highlights the problem that we have her in this country than the recntly approved Avastin, which I undrstand gives a patient maybe 6 months mor of existence for an astronomical price tag. If Happys free market model of health care existed, how many patients would find that an enticing proposition!

But you failed to answer my last query after your last post; are Canadians health suffering as a result of delays in testing and treatment? How many Canadians would choose to give up their current health care system? I actually like the idea that if you are in such a rush to have your test or procedure, then you can skip the que and pay out of pocket. The pure capatilistic system you seem to so desire!

Keep in mind that the guy who brought Canadian health care to fruition is considered a national hero!

You seem to prefer your rationing by ability to pay. The weak should simply suffer and die (or is your take that the stupid and lazy should suffer their just fate). Either way, it seems a cruel postion to take for someone in the healing profession.

Maybe doctors and health care execs should all just make less money so we can have a more affordable health care system. After all, I will bet most of us said we were not going into medicine for the money when we applied to medical school.

I think you missed the point of the article. CRP was thought to be a more poerful predictor of risk than LDL cholesterol. It did not necessarily state that it was the cause, but simply a marker for the presumed inflammation that occurs in the arteries of these patients. the more recent study suggests that CRP itself is not the protein causing the problem.

Being an internist who still enjoys admitting his patients and caring for them while hospitalized, I have seen incredible pressure from discharge planners to move "granny" out of the hospital as quickly as possible. The result is granny goes to the skilled care facility sicker than in previous years and is more apt to bounce back to the hospital as a result. Should the hospital who pushed granny out prematurely. Should they then be incentivised to continue this behavior if it benefits the hospitals bottom line, but negatively impacts the well being of the patient? I would think not.

Since health care entities want to act as silos of care rather than integrated entities, they optimise their payments by pushing aspects of patient care on to other providers. In this case, it is the SNF facility and the primay care doc who have to cope with caring for granny post discharge, while the hospital opotimises its DRG payment after discharging her with a 2 day stay, even though her chances of full recovery are better if she stabilizes in the hosptial for a few more days.

Our hospital used to have a transitional care unit for patients who were too tenuous to be transfered to a nursing home, which seemed like a great idea to me and was very popular with physicians and patients alike. Unfortunately, the hospital administrators did not seem to be as enthusiastic about such a facility, and when they needed a temporary place to house their oncology center while they spent a bundle building a shiney new oncolgy center, they chose to close this transitional facility for good. Probably because they found the daily SNF payment not lucrative enough for their taste. Furthermore with their payments not linked to whether the patient remains outside the hospital and improves, they can turn this responsibility over to the next silo, the nursing home. The nursing home then has to manage a sicker patient, for which they do not get any increase in reimbursement for, even if the patient requires a higher intensity of care.

To me it makes great sense to link and bundle the two so that hospitals will not continue to put undue pressure on docs to discharge their patient prematurely. Since more and more of your hospitalists friends are getting your paycheck directly from the hospital they work at, it will get harder to resist the wants and needs of well healed administrators who want to maintain their luxurious lifestyle. Sending granny packing as quickly as possible is one way they can do this. After all, if they sign your paycheck, it may get very difficult to resist their whims and desires.(Dr. Happy, we notice that your lengths of stay for grannys under your care is much higher than your peers. What are we going to do about this?)

I beleive I am coming to the realization that the way things work in medicine in this country, we really do not want competent comprehensive care physicians. Indeed the problem is that a competent MD does not need to refer as often to specialists and can depend on their experience to make more judicious choices in test ordering. In other words, they work to the contrary nature of profit driven medicine whose model fits that of every other buisness that flourishes by selling more and more of what you market.

As you have pointed out, this model works well for hospitalists in the hospital enviorment, since Medicare in its wisdom (lets give them some credit for once) has aligned hospital effeciency and payment to actually encourage the right behavior. Thus a knowledgeable hospitalist can save the hospital money. However, in the outpatient world, it is better to have an ignorant physician who refers to specialists like crazy and whose patients do worse, thus requiring more expensive hospital and tertiary care. Possibly this is why our specialist colleugues see the great benefit of NPs and PAs!

Until the correct incentives are created on the outpatient side of the system, we will continue to see little effort given to keeping the patient healthy in the first place. And we will continue to see the dumbing down of the entry point of care in the hopes that it will continue to feed and nourish the more costly hospital and specialty side of medicine. Its difficult to see how a more stupid and wasteful system could be created, but we have succeeded well in this respect. Now with all the highly compensated interests and their influence on goverment decision making, it will be a monumental task to see how this will change until it becomes a crisis.

some of my patients tell me that too, but I have a feeling that many may not think much of my talents or lack thereof. I o not doubt you are very competent internist/hopspitalist by quality of your writings, but patients who tell you you are a great doc, while nice to hear, is not a very scientific assmt. of your abilities.

I am constantly remined of the attending when I was a resident who was the worse doc in the hospital, but his patients all raved about him. Probably means we can be great salesmen but lousy docs at the same time.

Keep on writing great stuff but don't fall into the Doc as God complx that many ocs seem to posess.
