I think the Bible is not perfect. But I also get a lot of good out of it. I am lucky that my Christian "Baptist" Parents were not legalistic. So I grew up pretty happy. I used to be an atheist when I was a kid but not as well informed as you people. I have found that the atheists I run into know more about the Bible than the average christian. Ironic. Don't ya think? Anyway I have been down the Calvinism, Arminian road and now I am a Universalist. Who knows maybe I'll be an agnostic next. I don't think I will ever be an atheist. It just seems to me the complexity of the universe tells me there is a creator. Not a GOOD Christian God or a Bible God. But at least a creator. Thank you atheists for challenging christians on our stupid beliefs. We need to be challenged. I am glad we live in a place where Christians are not allowed to burn people at the stake for disagreeing with us. Anyway I thought you would get a kick out of this: I was thinking one day and I made a list of BAD things God made and I don't know why. What do you guys think about christian theodicy? And do atheists have a theodicy? Here is my list. Is is not complete it just came to mind. If, as you suggest, it is because of lack of competition on the insurance side, then the usual market response is that the insurer could drive the price of an MRI down in a saturated market of MRI providers (which is what we have).&lt;br /&gt;&lt;br /&gt;Now I could accept that insurers are happy to keep the prics higher, because the number they track in being profitable is the percent they pay out in medical claims.  The higher the percentage, the less they have to pay stockholders and executives.  If the market bears higher prices for insurance, which it has till now, then they stand to make more paying 1000 dollars for an MRI vs 500 dollars.  If their medical loss ratio is 80%, then simple math tells us that they get to keep 200 dollars for the 1000 MRI vs 100 dollars for the 500 dollar MRI.  Of course they have to keep charging more for medical insurance which seems to have been a successful strategy the past few years.  This may explaiun why they seem so willing to pay hospitals more for these type tests when they could be performed for much less cost in an outpatient facility.&lt;br /&gt;&lt;br /&gt;Karen,&lt;br /&gt;&lt;br /&gt;Of course Canadians don&amp;#39;t WANT to wait for their test.  But I can tell you if I refer a patient to a rheumatologist at my hospital, they will be offered an appointmnt in three months.  And I can think of several other examples where wait times exist just in my world, which is, by the way, at a very well funded hospital and health care system.  Secondly, what about the patients who have limited or no access to cancer screening and the examples of women who do not get proper pap sceening or mammograpy? And your example of colonoscopy waits; I dare you to try and get a screening colonoscopy AT ALL if you have no insurance in this country.  You currently have to wait more than 6 months for a mammogram at the county hospital here in Chicago.&lt;br /&gt;&lt;br /&gt;As to my oncology friend, length of survival is but one parameter which you have conveniently picked.  More important is months of MEANINGFUL survival.  Who wants to sign up to live an extra 6 months while being bombarded with highly toxic chemotherapeutic agents while you suffer a slow but painful death? And the NHS in Britain allows you to obtain private healthcare if you wish (but few do) and does not restrict you from receiving any form of therapy unless it is not approved by their equivalent of the FDA, which is no diffrent from what we have here.  Nothing highlights the problem that we have her in this country than the recntly approved Avastin, which I undrstand gives a patient maybe 6 months mor of existence for an astronomical price tag. The result is granny goes to the skilled care facility sicker than in previous years and is more apt to bounce back to the hospital as a result. Should the hospital who pushed granny out prematurely. Should they then be incentivised to continue this behavior if it benefits the hospitals bottom line, but negatively impacts the well being of the patient? I would think not.&lt;BR/&gt;&lt;BR/&gt;Since health care entities want to act as silos of care rather than integrated entities, they optimise their payments by pushing aspects of patient care on to other providers.  In this case, it is the SNF facility and the primay care doc who have to cope with caring for granny post discharge, while the hospital opotimises its DRG payment after discharging her with a 2 day stay, even though her chances of full recovery are better if she stabilizes in the hosptial for a few more days.&lt;BR/&gt;&lt;BR/&gt;Our hospital used to have a transitional care unit for patients who were too tenuous to be transfered to a nursing home, which seemed like a great idea to me and was very popular with physicians and patients alike.  Unfortunately, the hospital administrators did not seem to be as enthusiastic about such a facility, and when they needed a temporary place to house their oncology center while they spent a bundle building a shiney new oncolgy center, they chose to close this transitional facility for good. Probably because they found the daily SNF payment not lucrative enough for their taste. Furthermore with their payments not linked to whether the patient remains outside the hospital and improves, they can turn this responsibility over to the next silo, the nursing home. The nursing home then has to manage a sicker patient, for which they do not get any increase in reimbursement for, even if the patient requires a higher intensity of care. &lt;BR/&gt;&lt;BR/&gt;To me it makes great sense to link and bundle the two so that hospitals will not continue to put undue pressure on docs to discharge their patient prematurely. Since more and more of your hospitalists friends are getting your paycheck directly from the hospital they work at, it will get harder to resist the wants and needs of well healed administrators who want to maintain their luxurious lifestyle. Sending granny packing as quickly as possible is one way they can do this. After all, if they sign your paycheck, it may get very difficult to resist their whims and desires.(Dr. Happy, we notice that your lengths of stay for grannys under your care is much higher than your peers. Indeed the problem is that a competent MD does not need to refer as often to specialists and can depend on their experience to make more judicious choices in test ordering.  In other words, they work to the contrary nature of profit driven medicine whose model fits that of every other buisness that flourishes by selling more and more of what you market.  &lt;BR/&gt;&lt;BR/&gt;As you have pointed out, this model works well for hospitalists in the hospital enviorment, since Medicare in its wisdom (lets give them some credit for once) has aligned hospital effeciency and payment to actually encourage the right behavior.  Thus a knowledgeable hospitalist can save the hospital money.  However, in the outpatient world, it is better to have an ignorant physician who refers to specialists like crazy and whose patients do worse, thus requiring more expensive hospital and tertiary care.  Possibly this is why our specialist colleugues see the great benefit of NPs and PAs!&lt;BR/&gt;&lt;BR/&gt;Until the correct incentives are created on the outpatient side of the system, we will continue to see little effort given to keeping the patient healthy in the first place.  And we will continue to see the dumbing down of the entry point of care in the hopes that it will continue to feed and nourish the more costly hospital and specialty side of medicine. Its difficult to see how a more stupid and wasteful system could be created, but we have succeeded well in this respect.
