If you have ever written a large switch statement or have seen code with large switch statements, then chances are the developer could have used a state machine but failed to realize it.

This is why EA had to implement there own version of the STL. Alogithms using the STL can have different performance metrics because the STL is left up to the implementer of the compiler. When you are writing a game for Nintendo Wii, X Box 360, PS3, and Microsoft Windows--you don't want to have to fix run-time issues with your algorithms when moving between different compilers.
I have found that applications using using pre C++11 are usually designed with lots of unneeded "copying" (and yes, they are using pass by reference)--this is why move semantics were introduced.
