Having a Federal Reinsurance program similar to what large employers do with stop loss limits per person annually and lifetime would be the simplest solution and the one with 100% actuarial accuracy that would allow insurers to price with confidence.

The risk corridors that the R's deleted are the real reason PPACA is failing and some form of that using reinsurance/stop loss would work well.

Couple that with the full transparency of all medical/drug costs like jimbino below advocates would push the envelope from the consumer side.

We see calories in all we eat now at any restaurant from fast food to any other...we comparison shop on the web all the time...make the info transparent and accessible and consumers will drive change

Allowing "young seniors", ie 55-64 into a version of Medicare would be a huge help as well, lowering standard pool rates and also helping lower medicare rates as well.
