Our rural, outpatient physical therapy clinics are among the most profitable in our entire network. I speak to these owners on a daily basis and they are almost oblivious to the challenges facing urban clinics in most states.

Declining reimbursement is not the biggest challenge medical providers in general face, I believe. Over-capacity in urban areas and heavy competition are probably bigger factors than declining reimbursement for our firm (FYZICAL LLC, a physical therapy clinic franchisor).

Some urban practice owners are barely paying their own salaries while the rural owners are taking home $200-300k.

His thesis was credible until paragraph seven when he said the Congress would permanently eliminate the Sustainable Growth Rate formula next year.

Unfortunately, this statement demonstrates he is in the same camp as everybody else who claims to have a unique perspective on future events. 2014 was the year to fix the SGR because of a one-time revision to the 10-year cost estimate - it was half of what it had cost in 2013.

Now, the cost is probably baked into future spending. It no longer seems discounted and the Congress will have to find new offsets, or new taxes, to pay for it.

The article you reference says "Possible decreased quality of care" in the Abstract.

That is not "obviously and extremely well documented".

Hi Allan,

Is it possible for small companies or clinics to brand themselves? That's what really matters, right? How can I get more patients into my clinic instead of going to my competitor down the street.

Brands are the stuff of large corporations with annual marketing budgets. Brands take ongoing maintenance and annual investment to remain top-of-mind.

Many small and mid-sized clinics have mission statements and tag lines but I can't think of a single national or regional brand successfully implemented by a small business.

I'd be happy to hear your suggestions.

Lance is guilty. Sorry to say it but there's no other conclusion.

Look at it this way - what are the risks if the World Anti-Doping Agency is wrong? A false-positive drug test and the resultant accusations, media exposure, damage to the sport of cycling and eventual loss of credibility to WADA if the false-positive test result is exposed by an independent laboratory.

Therefore, the WADA sets the thresholds for the blood tests very high. The risk is an acceptable number of false-negatives. In other worlds, some cyclists who truly are dopers don't get caught and may win medals or cash sponsorships.

The public don't understand the fact that a "test" may have differing levels of reliability which is why the standard defense for a cyclist when accused is to deny, deny, deny.

You can't deny the test results - after all its YOUR blood. But, you can deny the causal chain. Alberto Contador claimed he ate a spiked steak that contained Growth Hormone and that's how it got into his blood.

Bottom line, there is no agenda against Lance. Just evidence.
