I think that a niche product with little competition could potentially be successful, however, I don't think that taking the leap of Entrepreneurship is the right way to go about getting to your ultimate goal.&lt;/p&gt;&lt;p&gt;In my opinion, the method with the highest probability of success would be to seek out the firm(s) that are currently in your chosen field (green energy/efficiency/et cetera) and who are just plain KILLIN' IT in their niche or in their product category. I think you ought to work for them for at least a year so that you can get an insider's view of what they are doing right (so that you can replicate it or mutate it) and what they are doing wrong or not doing at all (so that you can break off and offer those products or services that are currently missing in the marketplace). Yes, incentives to be healthy and not get injured do not necessarily exist. The costs are all paid by either the employer or the government. That doesn't incentivize responsibility for one's own health.&lt;/p&gt;&lt;p&gt;That's something that could use some change.&lt;/p&gt;&lt;p&gt;In my opinion, a bigger driver a cost is (you'll know you're listening to an economist in 3...2...1...) supply &amp;amp; demand. Supply of healthcare service is constrained by a regulatory scheme that REQUIRES a doctor on staff to direct the already-quite-capable nurses and does not allow for lower-expertise clinics staffed by RNs or ARNPs to proliferate and service those who cannot afford a doctor's care. We should be free to demand that kind of care if that is the level of care that best suits us.&lt;/p&gt;&lt;p&gt;The other big constraints are the policies pursued by the AMA/AAMC/AOA, organizations which restrict the number of medical colleges so that there is a permanent shortage of doctors, driving up their wages.&lt;/p&gt;&lt;p&gt;In Free to Choose, Friedman said "The AMA has engaged in extensive litigation charging chiropractors and osteopaths with the unlicensed practice of medicine, in an attempt to restrict them to as narrow an area as possible." (Wikepedia, "American Medical Association: Criticisms")&lt;/p&gt;&lt;p&gt;As a side note to this point about the AMA/AOA's attempt to limit the supply of doctors, the mid-level medical providers like Physician's Assistants and Nurse Practitioners are likewise bottlenecked because their own professors that train them are underpaid, driving down the supply of advanced nursing/P.A. professors and hence limiting the number of NP/PA graduates.&lt;/p&gt;&lt;p&gt;I think that the best way to keep healthcare costs at a reasonable level would be to break the AMA/AOA's lock on awarding M.D./D.O. degrees just as Ronald Reagan busted up the air-traffic controller union's grip on our air-traffic in the 80's.
