A friend of mine worked for a company that had the first initial last name template for email addresses. They wouldn't even make an exception for a new employee named Samantha Lavery. Poor woman had one of the most unfortunate email addresses ever.

Apparently, when I was 5 years old, my father needed some Preparation H at the corner store, and I went with him. Me being a helpful little chap, I ran over to the area and yelled at the top of my lungs, "So dad, do you need the ointment or the suppositories???" He said he wasn't sure whether to laugh uproariously or leave the store in shame.
